LCSW/LMHC
Case Manager•Hybrid to fill an opening with a Managed Care Organization in Pinellas and Pasco County.
Daily Responsibilities:
Assessments are Needs Based•not clinical and not diagnosing the members/providing treatments.
EX:
linking to providers, referrals for equipment, community recourses, groceries, tutoring Calls on average are under 30 minutes long. Candidates may have worked other positions that the calls are 45-60 minutes long but that is not the case for this. Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective care.
