• Assessment, evaluations; Dc planning with patients and families; Willing to step up to lead MDR discussions.

MINIMUM REQUIRED QUALIFICATIONS

• FL/compact RN license; BLS; 1+ year recent acute hospital case management experience.

PREFERRED QUALIFICATIONS

• Midas/Interqual/Meditech experience.
