The ideal candidate will play a crucial role in coordinating patient care, ensuring effective management of health services, and improving patient outcomes. This position requires a strong background in various areas of nursing, including infectious disease care, behavioral health, and primary care. As a Registered Nurse Case Manager, you will work closely with patients, families, and healthcare providers to develop and implement individualized care plans. Responsibilities Conduct comprehensive assessments of patients’ medical histories and current health status. Develop and manage individualized care plans that address patients’ specific needs. Collaborate with interdisciplinary teams to ensure coordinated patient care across various settings including clinics and hospitals. Monitor patient progress and adjust care plans as necessary based on diagnostic evaluations. Provide education and support to patients and their families regarding treatment options and health management. Maintain accurate medical records and documentation in compliance with regulatory standards. Engage in ongoing communication with healthcare providers regarding patient status, needs, and concerns. Experience Current Registered Nurse (RN) license in good standing. Experience in Home Health preferred. Experience with OASIS required. Familiarity with infectious disease management and behavioral health practices is a plus. Proficiency in case managing. Strong diagnostic evaluation skills to assess patient conditions effectively. Excellent communication skills to interact with patients, families, and healthcare professionals.
