ESSENTIAL DUTIES AND RESPONSIBILITIES
Coordinate the RAI process, assuring the timely completion of the MDS and CAAs, after evaluating the resident’s functional abilities and reviewing data to help the IDT develop a comprehensive, individual, resident-centered plan of care. Perform strategic scheduling considering Quality Measure Data Optimize revenue through the Assessment Reference Date selection for optimal placements within PDPM Case Mix Categories Work in collaboration with the IDT to identify and document the needs and preferences of the resident. Share responsibility for completion of assessment sections to staff possessing clinical knowledge about the resident such as social service and dietary personnel. Conceive and contribute to the development of the comprehensive care plan reflecting resident’s individual needs and preferences and monitor for necessary revision in between scheduled assessments. Lead clinical reimbursement meetings and help ensure federal billing requirements are met Participate in care plan conferences with residents and Resident Representatives’ JOB
REQUIREMENTS
Sound understanding of the RAI process and serve as a guide and provide support to other Interdisciplinary Team members Active RN or LPN license (Required) 1 years of previous MDS experience Working familiarity and understanding of PDPM (Plus, but is not required) Experience using MatrixCare software (Plus, but is not required)
Responsibilities:
- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for all residents in accordance with state and federal regulations
- Collaborate with interdisciplinary team members to gather necessary information for accurate and comprehensive assessments
- Ensure timely and accurate submission of MDS assessments to the appropriate agencies
- Monitor and update resident care plans based on MDS assessments and changes in condition
- Conduct regular audits to ensure compliance with MDS documentation standards
- Provide education and training to staff on MDS assessment processes and requirements
- Stay up-to-date with changes in regulations related to MDS assessments
