The ideal candidate will possess a strong medical background and a passion for delivering quality and comprehensive inpatient and outpatient care. Must be cognizant of, and comply with, all Center policies and procedures, as well as pertinent state and federal regulations. 3-5 days a week with some weekend availability. APRN must provide the full scope of Infectious Disease and or Pulmonary care services at nursing homes and long term care facilities, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance of their patients. We provide you with; a Scribe for notes, malpractice insurance, census planning and ongoing education. The company provides Nurse- Practitioners to Skilled Nursing homes in Florida to work with the facilities nursing team to improve quality care and reduce rehospitalizations. The company is currently supporting Skilled Nursing Facilities (SNF) all throughout Florida.
Responsibilities:
Provide direct patient care in various healthcare settings, such as nursing homes and long term care facilities Obtain health history and perform physical examinations Diagnose and direct counseling for each patient on a plan for treatment Prescribe medication in accordance with state statute and professional practice guidelines Maintains a problem-oriented health record for each patient Evaluation of new-admission visits Coordination of discharge visits Develop and implement patient management policies and procedures Communicate with attending physicians and/or specialists regarding patient care Conduct routine check-ups to patients to assess their health condition and discover possible issues Give appropriate advice for healthy habits (diet, hygiene etc.) and preventative actions to promote overall health Cultivate a climate of trust and compassion for patients Assures continuity of care by providing comprehensive medical services on a timely basis Provides excellent internal/external customer service via telephone, videoconference, chat or face-to-face contact to assist patients with their health care needs Follows established departmental and medical center policies, procedures, and objectives Monitor, assess, and document patient conditions and reactions to treatments. Collaborate with healthcare professionals to plan, manage, and assess patient treatments Educate patients and their families on proper healthcare procedures Ensure compliance with infection control policies and procedures Assist in medical procedures as needed

Sensous Planning Medical Specialty Infectious Disease Pulmonary Experience Nursing:
3 years (Preferred) Experience in Skilled Nursing Homes a plus but not mandatory License/Certification Advanced Practice Licensure (APN) with a Florida license Active license and DEA number (preferred) Previous experience in nursing or other medical fields. Ability to build rapport with patients and their loved ones. Ability to thrive in a fast-paced environment. Excellent written and verbal communication skills.
