Primary Responsibilities:
- Ensure continuity of care in both inpatient and outpatient settings within the guidelines of patient benefits and established contracts.
- Perform on-site or telephonic care management functions.
- Conduct discharge planning, utilization reviews, and support transitions from inpatient to outpatient care.
- Manage referrals, schedule follow-ups, and support telehealth visits.

Qualifications:
- High School Graduate or equivalent
- Current and unrestricted LPN licensure in Florida
- IV Certification and 1-2+ years of nursing experience in acute care settings

Pay Details:
- $30.
