Key Responsibilities:

Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team. Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated. Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques. Supervise and support LPNs and HHAs to ensure high standards of care. Maintain timely, accurate, and thorough clinical documentation. Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates. Facilitate staff training and education to promote quality improvement. Participate in on-call rotation, providing vital support to patients and families after hours as needed. Engage in professional development and continuing education. Perform additional duties as needed to support patients and team operations.

Qualifications:

Current Registered Nurse (RN) license in the state of Florida Minimum of one (1) year of clinical nursing experience Current CPR certification Valid driver’s license, reliable transportation, and up-to-date liability insurance
