Scrubs AllowanceHours:

This position will work in the field helpingpatients in their home setting.
- The office is in Roseville, MNand the general territory for this position of where the patientslive would be the southeast section of the Twin Cities Metroarea.

A few must-haves for thisposition: Graduate of an accredited school ofnursing and has a current license as a Registered Nurse in theState of MN.Able to travel within a geographic area serviced by theoffice. Reliable transportation, valid drivers license, and proofof auto insurance is required.
CPR Certification is requiredTwo (2)years of current nursing experience preferred.
One (1) year casemanagement experience preferred.
Preferred:
- Community health and awide range of Medical-Surgical experience.
Some functions of whatyou would do as a
RN Case Manager:
Perform
- assessments and procedures,which require
- knowledge, judgment, and nursing skill based uponprinciples of psychological, biological, physical and socialsciences.
Visits in home clients to evaluate the quality of carebeing provided and review
- clinical notes.
Participate in ongoinginterdisciplinary assessment of the patient.
Provides services thatare ordered by the physician as indicated in the plan ofcare.
Counsels and educates the patient/client and family in meetinghealthcare related needs.
Completes timely and accurate clinicalnotes including addressing the patient’s/client’sprogress.Effectively communicates with other members of theinterdisciplinary healthcare team providing care to promotecoordination of patient/client care and planning for discharge.
