Job Description:

Breezy Hills Rehabilitation and Care Center is seeking a dedicated and detail-oriented RN MDS Coordinator to join our care team. The ideal candidate will be responsible for managing and completing all aspects of the MDS process, ensuring compliance with Medicare, PDPM, and state/federal regulations while promoting exceptional resident care.

Key Responsibilities:

Coordinate and complete MDS assessments accurately and on schedule. Develop, implement, and update individualized care plans. Ensure documentation supports PDPM and Medicare billing requirements. Collaborate with interdisciplinary teams to provide high-quality care. Analyze QI/QM data and support performance improvement initiatives. Assist with preparation for surveys and audits. Ensure timely electronic transmission of all assessments. Provide training and support on MDS-related updates and best practices.

Requirements:

Active RN license in the state of Florida. Strong knowledge of PDPM and Medicare processes required. Minimum 2 years of experience in a skilled nursing facility preferred. Proficient in MDS 3.0 and RAI guidelines. Excellent communication, problem-solving, and team collaboration skills.
