- Hybrid work We are seeking a compassionate and skilled Private Duty Nurse to provide personalized care to patients in their homes or designated settings.
The position will focus on integrating disease management and lifestyle interventions into the care planning process. Working with physicians in managing more highly skilled home care needs.
Duties include:
Conducting admissions Home visits Community education And much more. This position will work closely with the CEO in the implementation of a more progressive home care model responding to the changing and challenging needs of aging-in-place.
