POSITION SUMMARY

Caya Health is seeking a qualified Family Nurse Practitioner to be responsible for providing family-focused care in our outpatient facility located in Lake Mary, Florida. You will be responsible for developing treatment plans for acute and chronic diseases, educating patients on disease prevention, performing diagnostic tests, and managing the overall patient care. In addition to responsibilities such as writing prescriptions and ordering tests, the FNP is required to provide primary, acute, chronic, and specialty care for illnesses affecting any member of the family, and should display a professional, yet nurturing personality while consistently providing excellent patient care with a strong emphasis on wellness and disease prevention.

ESSENTIAL POSITION DUTIES

Assessment of the physical, emotional, and developmental status of the individual or family across the life span. Diagnosing and treating acute and chronic conditions based upon assessments, clinical judgments, and intervention to maintain, restore and /or improve the health status of the individual and/or family. Performing physical examinations. Managing patient care and maintaining patient records. Collaborate and/or coordinate with other health care providers and/or community services to maximize the client’s health status. Develop treatment plans and evaluate the outcomes of prescribed treatments and plans of care. Educating patients about healthy lifestyle choices. Knowledge and experience with medical weight loss preferred but not necessary.

COMPLIANCE RESPONSIBILITIES

Complies with applicable legal requirements, standards, policies and procedures including, but not limited to, those within the compliance process, Standards of Conduct, HIPAA, State Licensure and JCAHO. Participates in required orientation, ethics and HIPAA training programs. Reports concerns and suspected incidences of non-compliance following Standards of Conduct. Ensures compliance with all state, federal, and local laws regarding licensure and certifications of staff. Ensures proper maintenance of medical records as prescribed and in compliance with local, state, and federal laws. Stays informed about changes in the field of nursing and shares information with appropriate staff and administrative personnel. Participates in the development of policies and procedures. Participates, as requested, in process improvement responsibilities. Responsible for other duties as assigned.

PROFESSIONAL EXPERIENCE / EDUCATION REQUIREMENTS

Graduate of an accredited school of Advanced Registered Nurse Practitioner. Master’s degree in nursing. Must be Board Certified Florida licensure & certification. Ability to work and interact with departments throughout the organization. Able to handle multiple projects concurrently. Must be flexible and enjoy working in a changing environment. Respect for confidentiality & maintains ethical conduct. Effective interpersonal, verbal, and written communication skills.
