Job Description
RN | Skin Care | PRN St. Augustine, Florida Nursing 51776 The Wound Care RN provides consultation and treatment of wounds, pressure injuries and ostomies. The Wound Care RN interacts with multiple disciplines, working closely with nurses, allied health clinicians and providers to deliver services to patients in all areas of the hospital.
Responsibilities Wound Assessment and Treatment:
Conduct comprehensive assessment of wounds, including pressure injuries, surgical wounds, burns, diabetic ulcers, and other acute or chronic wounds Identify the type, severity and stage of wounds and document findings accurately in the electronic health record. Develop and implement individualized wound care plans based on evidence-based practice.
Wound Care Management:
Perform wound care and application of appropriate dressings or wound care products. Monitor wound healing progress and adjust POC accordingly. Apply, monitor and manage negative pressure wound therapy.
Ostomy Assessment and Management:
Conduct assessment of new and existing ostomies and educate patient, caregiver and staff on appliance and management of.
Patient and Family Education:
Educate patients and caregivers on proper wound care techniques, prevention of infection and lifestyle adjustments to promote wound healing. Provide guidance on nutrition and hydration to support wound healing and refer to nutrition services when necessary.
Collaboration with interdisciplinary team: Collaborate with providers and other allied healthcare professionals to optimize patient outcomes. Provide recommendations on wound management to the healthcare team and participate in discharge planning.
Documentation and Reporting:
Maintain accurate, detailed and timely documentations of wound assessments, treatments and patient responses. Maintain and report on hospital acquired pressure injuries. This includes drilldowns and preventive measure/actions plans.
Staff Education and Training:
Provide training and education to nursing staff on best practices in wound care and prevention including new employee orientation. Serve as a resource for wound care questions and support staff in implementing policy and procedure as it relates to wound and ostomy care.
Performs all other duties as assigned by management within job scope.
Qualifications Required education:
Associates in Nursing Required experience: 2 years of nursing experience Wound Care certification. Required on hire or can be obtained within the first 12 months of hire.

Certified Wound Care Specialist (CWS) or Certified Wound Care Associated (CWCA)
