POSITION DUTIES

•Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS

•FL/compact RN license; BLS; 2-4 years recent RN CM experience in acute care facility; Meditech / Midas experience

LENGTH OF ASSIGNMENT

•13 weeks

SHIFT / HOURS PER WEEK

•0800-1630 m-f and every other weekend SYSTEMS•Meditech / Midas
