Daily Responsibilities for
Clinical-Care Manager :
Travel:
Will receive IRS mileage rate
- about 50% travel Will collaborate with members to schedule home visits. They have a mixed Tier 3 & 4 members as a weighted caseload of 200 members: Tier 3
- members are called 1x per month and field visits 1x per quarter and these members are less than 1/2 their case load. These are the more complex members which is why they need to be seen on onsite. Will be larger majority of their caseload. Tier 4
- members are only called 1x per quarter and there is no field visits except for the initial assessment will be the only time they have a home visit. If a member comes up on an Ad-Hoc report for an emergency visit they will have to call that member to check in Keeping the members consistent
- could have them from birth to when they outgrow the program May need to call providers to ensure they are still on Sunshine’s plan before providing to a member
Field Visits:
Not an hour long at home. The candidate can meet the member where the member would like to meet which could be at home, at a doctor’s appointment, park, etc as long it is HIPAA compliant. Sometimes these visits are 5-10 minutes as they are addressing the members needs and it could be very brief, or could be longer if the member has more needs
Frequency:
The candidate will have control over when they do their field visits (during business hours) so for example they can complete all their field visits in 1 day and the rest of the week they are WFH. It is infrequent for field visits and are usually just several per week and majority of the time they will be at home. Average is 1-2 visits a week
