Local Contract Essential Job Functions 1. Works with the providers and clinic staff to identify patients with chronic disease diagnoses. 2. Supports longitudinal care of the patient with stable chronic care conditions by: a. performing focused assessment of chronic care conditions b. performing medication reconciliation c. conducting Motivational Interviewing and Self-Management Goal setting d. providing patient education using standardized, approved educational materials e. creating referrals to appropriate agencies and resources 3. Supports transition of the patient with stable chronic care conditions from inpatient to outpatient setting, by: a. performing focused assessment of transitional needs b. performing medication reconciliation c. reviewing contingency plan d. providing patient education using standardized, approved educational materials e. assisting with post discharge needs such as prescriptions, transportation, Durable Medical Equipment (DME), appointments 4. Notifies providers and/or supervising RN of changes to patient’s health condition 5. Performs accurate and timely documentation in the electronic medical record 6. Participates in daily huddles and Patient Care Coordination (PCC) meetings 7. Prepares accurate and timely reports, as required, for weekly meetings 8. Maintains continued competence in nursing practice and knowledge of current evidence based practices 9. May perform clinical tasks within their scope of practice 10. Performs all other related duties as assigned Minimum Required Education, Experience & Skills

• Registered Nurse with an active and unrestricted license to practice in the state of employment
• Current BLS certification
• Two or more years of experience in a physician’s office, clinical or hospital setting
• Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
• Excellent verbal and written skills
• Strong interpersonal skills
• Ability to interact productively with individuals and with multidisciplinary teams
• Proficient computer skills to work efficiently with electronic medical records
• Excellent organizational and prioritization skills Preferred Education, Experience & Skills
• Experience related to patient education and/or motivational interviewing skills and self management goal setting
• IV Certification
