Annual Wellness Nurse 3.5 3.5 out of 5 stars Savannah, GA 31405 SouthCoast Health is looking for a Full-time Annual Wellness Nurse for our Chatham Center Location Responsible for providing professional nursing care while conducting annual wellness visits to our clinic patients under the general policies and procedures established by SouthCoast Health. Will participate in providing health promotion, health maintenance and management of illnesses for patients.
Requirements:
Current Georgia LPN licensure. Maintain current BLS/CPR Certification.
Qualifications s:
Ability to exercise professionalism in dealing with all levels of personnel, patients and patients’ families. Ability to communicate clearly. Ability to learn and retain information regarding patient care procedures. Ability to complete tasks as directed by physician and nursing staff. Ability to deal with many tasks at one time with flexibility. Knowledge and demonstrated proficiency in performing clinical skills. Knowledge of medical charge reporting/records.
Responsibilities:
Conducts Annual Wellness Visits (AWV) according to guidelines set forth by Medicare and ensure they are within insurance plans timeline. Document Annual Wellness Visits in the patient’s electronic medical record. Conduct the functional assessment to evaluate patient basic life skills and documents in electronic medical record. Use motivational interviewing strategies to initiate advance care planning. Assist patient with completing Advance Directive form. Maintains confidentiality in matters regarding the clinic, its patients and employees. Communicates with pharmacy. Coordinates care with hospital, home health services, hospice or other institutional settings per provider’s order. Performs clinical and non-clinical duties as assigned by Supervisor. Assists in other physician areas as assigned by Supervisor.
Education/ Experience:
Minimum of one year clinical experience, preferably in a hospital or office practice setting.
LPN:
Preferred-Graduate of an accredited school of practical/vocational nursing program.
MA:
High school graduate or equivalent required. Graduate of an accredited Medical Assistant training program. Medical Assistant in the military (DD214), or EMT-A or paramedic is required.
