Schedule:

M-F (8:30am-5pm) Our established and committed Home Care agency is seeking a Nurse Coordinator to join our team at our Savannah, GA office. We provide a range of services, including medical home health care, non-medical home care, infusion, and pharmacy services, with a focus on supporting residents who need assistance with daily activities or have physical or developmental disabilities. Qualifications:

Previous experience in administrative/ medical setting Highly detail-oriented with a reliable, positive, and punctual work ethic. Strong organizational, multitasking, and communication skills. Ability to manage both patient care and administrative tasks with a customer service focus. Tech-savvy with experience in using scheduling and health care management software.

Responsibilities:

Coordinate and Maintain accurate records of patient care , including care plans, schedules, and medical updates. Ensuring quality care is being provided and services are delivered on time. Create and manage monthly staff schedules for nurses, and pharmacy delivery personnel Act as a liaison between patients, their families, and the healthcare team, ensuring communication is clear and consistent.
