(HT) Summary We are seeking a dedicated Registered Nurse-Case Manager to join our team at ATC Healthcare. As an RN Case Manager, you will play a pivotal role in managing and coordinating the care of patients with complex or chronic medical conditions.
Responsibilities Assessing Patient Needs:
Conduct comprehensive assessments to determine patient needs and develop individualized care plans.
Care Coordination:
Collaborate with healthcare teams, patients, and their families to ensure timely and appropriate healthcare services.
Patient Advocacy:
Act as a dedicated advocate for patients, ensuring they receive the best possible care and resources.
Monitoring Progress:
Regularly evaluate patient progress and adjust care plans as necessary to achieve optimal outcomes.
Education and Support:
Educate patients and their families about care plans, health care options, and provide emotional support. Requirements At least 1 year experience in the above field. Current Registered Nurse (RN) license in the state of practice. Active BLS/CPR and TB certifications. Strong knowledge of anatomy, physiology, and medical terminology. Experience in critical care or emergency nursing is preferred. Excellent communication skills for effective interaction with patients and healthcare teams. Ability to work well under pressure in fast-paced environments. Experience working with children or individuals with developmental disabilities is a plus. Additional Information Shift Monday
