This role requires OASIS proficiency and experience with Homecare Homebase, making it a perfect opportunity for skilled RNs to provide exceptional care in the comfort of patients’ homes.

Key Responsibilities:

Conduct thorough patient assessments and document findings using OASIS Develop personalized care plans based on patient needs and goals Administer medications, perform wound care, and provide patient education Work with an interdisciplinary team to ensure coordinated care delivery Support patients and families in managing care effectively at home

Qualifications:

Active RN license in Georgia Proficient in OASIS documentation and experienced with Homecare Homebase Minimum of 2 years of Home Health nursing experience preferred Ability to pass a background check, drug screening, and onboarding in compliance with Joint Commission standards Open to both local and travel candidates
