The successful candidate will join our client, a leading nationwide home health agency, and provide quality patient care in a home-based setting.

Hours:

Full-time, 40 hours per week, Monday-Friday, 8am-5pm, with on-call Monday-Thursday for potential call outs, and one weekend per month

Compensation:

Negotiable, based on experience and territory budget

Mileage Reimbursement:

.48c visit to visit; estimated 150-200 miles per week

Available for travelers

Productivity:

25-32 points per week Job Responsibilities Develop and implement individualized care plans, coordinating with physicians, physical therapists, and other healthcare professionals. Administer medications, monitor vital signs, and provide skilled nursing care, including wound care, IV therapy, and catheter care. Educate patients and their families about care plans, medication management, and other health-related topics. Maintain accurate patient records in Homecare Homebase (HCHB), ensuring compliance with regulatory requirements. Travel to patient homes within the territory, using reliable transportation. Participate in interdisciplinary team meetings and case conferences to ensure coordinated and effective patient care. Adhere to Joint Commission standards and other regulatory requirements. Requirements Active Registered Nurse (RN) license in the state of Georgia. Minimum of 1 year of home health experience. Minimum of 1 year of hospice experience. Proficiency in Homecare Homebase (HCHB). Current American Heart Association or American Red Cross Basic Life Support (BLS) certification. Reliable transportation with valid driver’s license. Ability to meet Joint Commission onboarding standards upon offer.
