Capstone Hospice is in search of a registered nurseto join our dedicated hospice team in the Tucker, Stone Mountain
- Lithonia territory. This position is full time with expectations ofworking Monday
- Friday, 8am
- 5pm.
The RN Case Manager will managepatient care to a designated case load of patients that live intheir personal home and assisted living facilities. The RN CaseManager will work independently and be responsible for themanagement of the hospice patients’ symptoms, pain, familyeducation, pronouncements and following regulatory documentationand is required to attend bi-weekly interdisciplinary groupmeetings in the office. RequirementsCapstone Hospice iscurrently seeking a Full Time Hospice RN to manage a designated apatient case load in the Decatur, Stone Mountain, Atlanta-Dekalbarea. The functions of this role will require the Hospice Nurse tomanage a typical case load of home and community-based hospicepatients, Monday
- Friday, this is a day shift requiring the nurseto be available 8:00am
- 5:00pm.
Essential responsibilities of therole:
Observe signs and symptoms and reports to the physician andother team members any unexpected changes in the patient’s physicalor emotional condition.
Teaches, supervises and counsels the Hospicepatient and family members about providing care for thepatient.
Organizes work schedule and utilizes time management to beable to attend all required meetings.
Develops and re-evaluates thepatient/family care plan in conjunction with IDG to meet needs andmaintain continuity of care.
Documents visits in electronic medicalrecord system (Wellsky Consolo)Performs specific nursing proceduresas needed (e.g., treatments, management of symptoms) followingdoctor’s orders.
Observes signs and symptoms and reports to thephysician and IDG members any unexpected changes in the patient’sphysical and emotional condition.
Attends teamconferences.
Coordinates the implementation of the plan of care forpatients residing in SNF or ALF.Works with interdisciplinary groupconcept of patient care.
Follows the policies and procedures ofHospice. Observes confidentiality and safeguards allpatient-related information in compliance with the HIPAAregulations.
Complies with infection control policies andprotocols.
Candidates must meet the following qualifications andexperience to be considered:
Graduate from an accredited school ofnursing.
Currently licensed as a registered nurse to practice in thestate of Georgia.
Minimum of 2 years as a registered nurse.
Activepatient contact within the past three years to include hospice.
Mustprovide evidence of annual TB test and other state required testsor examinations.
Must have current valid driver’s license, autoliability insurance and your own reliable transportation.
To besuccessful in this role you should have the following knowledge &skills:

Ability to supervise others appropriately.
Abilityto respond appropriately to crisis outside of a hospitalsetting.
Acceptance and adaptability to different social, racial,cultural and religious modes.
Have completed or the ability tocomplete a designated Hospice training program.
PositionFactors:
Physical Requirements:
Requires considerable physical effortmost of the day including, kneeling, squatting, reaching, twisting,climbing, walking, exposure and humidity changes and maximal assistin lifting and/or transferring of a 100-pound patient.
Must possesssight/hearing senses or use appropriate adaptive devices that willenable senses to function at a level required to meet the essentialduties of the position.
Mental Requirements:
Must be able to workindependently, make judgements based on assessments and dataavailable and act accordingly.
Must be flexible, innovative, andpossess good interpersonal skills.
Must be able to cope with mentaland emotional stress and demonstrate emotional stability.
WorkingConditions:
Be able to tolerate exposure to elements including, butnot limited to, odors, blood, body fluids, excrements, adverseenvironmental conditions and hazardous materials.
