JOB SUMMARY

RN Case Manager is responsible for providing in home medical support to families. The RN Case Manager will develop medical teaching plans, complete weekly health care screenings, support and advocate for child/family by attending doctor appointments, hospital visits, and ensuring appropriate communication between the foster family and medical staff in the community as necessary. RN Case Manage will be required to provide in home services to foster families that are caring for medically complex children that are in the custody the Division of Family and Children Services. This position requires extensive driving around the Metro Atlanta area. This is an ideal position for someone who is passionate about team-based work for children and families with significant special needs.

QUALIFICATIONS

A valid Georgia Driver’s License; have reliable transportation and carry vehicle insurance with 100/300/100 limits. Clean driving record is a requirement. Must be an RN licensed to practice in this state with a minimum of two (2) years healthcare related experience. Must be able to relate positively and favorably to the patients, families and outside agencies and to work cooperatively with other employees. Experience working with children with special healthcare needs; Must have excellent verbal and written communication skills’
