Job Summary:

The RN Evening Admissions Nurse will be responsible for managing the entire admissions process for new residents during the evening shift. The ideal candidate will have extensive experience in nursing home admissions and be able to efficiently admit 4-6 new residents daily. This role requires an individual who can multitask effectively, manage a high volume of work, and collaborate seamlessly with the interdisciplinary team to ensure smooth transitions for new residents.

Key Responsibilities:

Conduct and manage the admission process for 4-6 new residents per day, ensuring that all required documentation, assessments, and care plans are completed accurately and efficiently. Work closely with the interdisciplinary team (including social workers, physicians, and therapists) to ensure comprehensive, person-centered care plans are developed for each new resident. Provide clear and timely communication with residents, families, and staff throughout the admission process. Ensure that all medical records and documentation are completed in compliance with facility policies and regulatory requirements. Coordinate with various departments (such as dietary, pharmacy, and housekeeping) to ensure that residents have all necessary supplies, medications, and equipment upon admission. Monitor and assess the health condition of incoming residents during the admission process, ensuring that any urgent medical needs are addressed immediately. Work independently while managing a fast-paced workload and be able to handle multiple tasks simultaneously with accuracy and attention to detail. Maintain positive relationships with families and residents, providing clear and compassionate information about the admission process and their care. Serve as a resource and provide training for staff on best practices in the admissions process.

Qualifications:

Active RN License in the state of Georgia. 3+ years of nursing home admissions experience or similar experience in a long-term care setting. Proven ability to admit 4-6 residents per day while managing multiple tasks in a busy environment. Strong ability to multitask and prioritize effectively in a fast-paced setting. Excellent interpersonal skills with a focus on teamwork and communication. Ability to work closely with an interdisciplinary team to ensure smooth and efficient admissions. Knowledge of nursing home regulations and compliance standards. Ability to work evening shifts (typically 3 PM – 11 PM) and be flexible as needed. Strong organizational skills and a commitment to maintaining high-quality care standards.
