Overview:
•Sign-on Bonuses up to $10,000.00•As a HomeHealth Nurse, you will be responsible for providing nursing care toassigned patients through the use of nursing process in accordancewith standard nursing practice and established nursing policies andprocedures. Assumes full responsibility and accountability forcoordination of patient care; services, referrals and resources forassigned caseload. Supervises care rendered by Home Health Aides.

Qualifications:
Education:
BSN ispreferred.
Licensure/Certification/Registry:
RN licenserequired.
Current CPR certification required.
Will accept a MHSvc carfor business purposes only as deemed necessary bygeography/mileage. Must possess valid Illinois drivers license andmust be deemed as an acceptable driver in accordance with the MHSFleet Safety Policy (five year MVR will be required). Must haveavailability of personal vehicle with proof of insurance.
Meetscredit range for Clinical Ladder Levels as follows:
Clinical NurseI:
Registered Nurse who achieves a range of 0-9 credits on theClinical Ladder Behavior Grid.
Clinical Nurse II:
Registered Nursewho achieves a range of 10-19 credits on the Clinical LadderBehavior Grid with evidence of behaviors from at least twocategories of the Grid.
Clinical Nurse III:
Registered Nurse whoachieves a range of 20-29 credits on the Clinical Ladder BehaviorGrid with evidence of behaviors from at least three categories ofthe Grid. Minimum BSN, enrollment in a BSN or MSN program, ornational certification required.
Experience:
A minimum of two (2)years acute care clinical experience is preferred. Home healthexperience preferred.
Other Knowledge/Skills/Abilities:
Must be ableto position and/or transfer an average adult patient and possessgross coordination sufficient to perform patient care tasks asrequired.
Environmental working conditions include exposure toblood, bodily fluids/tissue, contagious diseases, animals,etc.
Responsibilities:
Professional Nursing CarePerform ageappropriate assessment utilizing critical thinking skills.
Functionas a case manager for a group of patients in a particulargeographic location.
Provide after hours care on a rotationbasis.
Develop and revise an individualized Plan Of Care inpartnership with patient/caregiver based on Identification ofpatient problems/need.
Implement specific interventions to attainoutcomes.
Communicate/collaborate with other disciplines to provideoptimal patient care and services.
Participate in interdisciplinaryteam meetings as appropriate.
Participate in educational offerings(ex: skills day; oxygen inservice; etc)Supervision andDelegationAppropriately supervise performance of LPN.Appropriatelysupervise performance of HHA and provide documentation to supportsuch.
