We are looking for acompassionate Home Health nurse for our Reedsburg territory. OurTerritory covers Sauk, Baraboo, Wisconsin Dells, Adams/Friendship,Lyndon Station, Mauston, New Lisbon, Richland Center, Muscada,Lavalle, Loganville, Reedsburg.

Schedule:

M-F 8a-4:30p plusoccasional call during the week. 2 Holidays/year!

Compensation:

$31.90-$51.04, dependent on years of experience .70/mile paid outin addition to paycheck every 2 weeks $10,000 sign on bonuseligible for qualified candidates paid out on first paycheck!

Job Summary:

Provides direct nursing care inaccordance with established policies, procedures and protocols ofthe healthcare organization.

Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES

Uses standards of medical-surgical nursingpractice to increase the quality of care and quality of life forthe patient, family, and significant other(s) by systematicallyevaluating the quality and effectiveness of nursing practice.

Establishes effective relationships and serves as an advocate withthe patient, family, and significant other(s) to facilitate thedevelopment of the care plan unique to the patient’s care needs.

Uses the nursing process to develop and implement the plan of care,collect assessment data for each patient, determine appropriatenursing diagnoses, identify expected patient outcomes, determinethe plan of care, implement interventions, and evaluate patientoutcomes. Develops assessment and management strategies based on aconsideration of the physical, functional, cultural, social,economic, developmental, spiritual, vocational, and leisuredimensions of human responses to actual and potential healthproblems. Demonstrates knowledge of the impact of actual orpotential illness and each patient’s physical, functional,cultural, social, economic, developmental, spiritual, vocational,and leisure status. Utilizes leadership skills by coordinating andcollaborating with the patient, family, significant other(s),communities, and members of the inter-professional team inassessing needs, setting goals, planning interventions, providingcare, and evaluating outcomes. Utilizes effective communication toparticipate with the patient, family, significant other(s), andother health care providers in collaborative decision making thatreflects the understanding that care should be culturallysensitive, ethical, legal, holistic, informed, compassionate, andhumane, and within the boundaries of available economic resources.

Educates the patient, family, and significant other(s) aboutmeasures that promote, maintain, and restore health or promotecomfort. Fosters a healthy work environment within the practicesetting and profession by serving as a mentor and role model fornursing colleagues, students, and others. Applies the existing bodyof evidence-based practice and scientific knowledge in health careto medical-surgical nursing practice, ensuring that nursing care isdelivered based on patient’s age-specific needs and clinical needsas described in the department’s Scope of Service. Works in aconstant state of alertness and safe manner. May perform point ofcare testing according to policies and procedures. Performs otherduties as assigned.

Graduate of accredited school ofnursing or education equivalency for licensing

PHYSICAL REQUIREMENTS

Constant use of speech toshare information through oral communication. Constant standing andwalking. Frequent lifting/carrying and pushing/pulling objectsweighing 0-25 lbs. Frequent sitting, reaching and keyboard use/dataentry. Frequent use of vision and depth perception for distancesnear (20 inches or less) and far (20 feet or more) and to identifyand distinguish colors. Frequent use of smell to detect/recognizeodors. Frequent use of hearing to receive oral communication,distinguish body sounds and/or hear alarms, malfunctioningmachinery, etc. Occasional lifting/carrying and pushing/pullingobjects weighing 25-50 lbs. Occasional lifting/moving of patients.

Occasional bending, stooping, kneeling, squatting, twisting,gripping and repetitive foot/leg and hand/arm movements. Occasionaldriving. Rare crawling and running.
