We are looking for acompassionate Home Health Nurse for our Reedsburg location. Nurse’scover territory of Sauk, Baraboo, Wisconsin Dells,Adams/Friendship, Lyndon Station, Mauston, New Lisbon, RichlandCenter, Muscada, Lavalle, Loganville, Reedsburg. Whole territorywill only be covered when on call.

  • Areas will be assigned.
  • 5regular visits in a 8 hour day.
Schedule:

Sat 8 am to Mon 8 am .Option to work Fridays! $5,000 sign on bonus for qualifedcandidates! $8.50 shift differential for WO in addition.

JobSummary:

Coordinates, facilitates and performs interdisciplinaryplan of care through assessment and evaluation of patient conditionand effectiveness of care under the direction of the attendingphysician by traveling to patient homes, Community-BasedResidential Facilities (CBRF), assisted living locations, hospitalsor other to meet patient and family/caregivers, assess patient careneeds, acuity in relation to policies, procedures and regulationsof home care and/or hospice services.

Job Responsibilities andRequirements:
PRIMARY RESPONSIBILITIES

Assesses and coordinatespatient’s discharge planning needs with members of the healthcareteam. May perform point of care testing according to policies andprocedures. Collaborates, consults and communicates with physicianregarding the plan of care and changes to patient status. Providesphysician with regular updates on progress and recommends revisionof plan care, documents status and communication in adherence withregulations. Coordinates and communicates with multi-disciplinaryteam members and outside agencies as appropriate to achieveexpected outcomes for patient care and education through end oflife or discharge, ensuring accurate and complete documentation andcoordination of unique care plan. Completes admission paperwork andverifies insurance information. Completes a comprehensive medicalassessment by entering data into the clinical computer program.

Ensures that patient assessment is completed in a timely manner.

Provides skilled nursing care within the established plan of care,including performance of tasks such as taking vitals, performingwound care, IV care, ostomy care, blood draws, injections, and thephysical assessment of the overall patient’s medical condition.

Documents visit notes, treatments, and care. Follows visit patternaccording to physicians orders and ensures required visits arecompleted in a timely manner. Ensures all documentation is timely,accurate and complete in adherence to regulations. Collaborateswith the patient, patient’s family and healthcare team to develop acomprehensive plan of care based on the unique needs of thepatient. Responds to calls from patients, family members andothers, completes follow-up to address needs, issues, or concerns.

Works in a constant state of alertness and safe manner. Performsother duties as assigned.

SSM EDUCATION

Graduate of accreditedschool of nursing or education equivalency for licensing EXPERIENCENo experience required

PHYSICAL REQUIREMENTS

Constant use of speechto share information through oral communication. Constant standingand walking. Frequent lifting/carrying and pushing/pulling objectsweighing 0-25 lbs. Frequent sitting, standing, walking, reachingand repetitive foot/leg and hand/arm movements. Frequent use ofvision and depth perception for distances near (20 inches or less)and far (20 feet or more) and to identify and distinguish colors.

Frequent use of smell to detect/recognize odors. Frequent use ofhearing to receive oral communication, distinguish body soundsand/or hear alarms, malfunctioning machinery, etc. Frequentdriving. Frequent keyboard use/data entry. Occasionallifting/carrying and pushing/pulling objects weighing 25-50 lbs.

Occasional lifting/moving of patients. Occasional bending,stooping, kneeling, squatting, twisting, gripping and repetitivefoot/leg and hand/arm movements. Rare crawling and running.
