Position Overview:

This role requires a Registered Nurse (RN) with unrestricted licensure in California and a strong background in clinical operations and medical management activities. The Case Manager RN will work independently to assess, plan, implement, coordinate, monitor, and evaluate patient care across the continuum, ensuring optimal outcomes..

Key Responsibilities:

Coordinate and deliver appropriate levels of care under the supervision of an RN or MD. Assess and interpret customer needs and requirements, providing effective solutions to non-standard requests and challenges. Utilize advanced clinical judgment to solve moderately complex problems and conduct analyses. Provide health education, coaching, and treatment decision support to members. Serve as a resource to others, offering guidance on difficult issues and explaining concepts in practice. Collaborate with members to move them along the continuum of change and influence their healthcare decisions.

Required Qualifications:

RN licensure in California. Minimum of 3 years of clinical RN experience demonstrating excellent clinical skills and judgment. Strong inbound and outbound telephonic communication skills. Proficiency with Microsoft Word, Outlook, Internet, and the ability to create, edit, and manage documents. Expertise in conflict management and the ability to prioritize responsibilities under pressure. Intermediate or expert ability to assess needs, motivate, and influence members.

Preferred Qualifications:

Bachelor’s degree in Nursing. Experience in coaching and decision support.
