Hospice RN Alpha Comfort Care 1745 Phoenix Boulevard, Atlanta, GA 30349 Overview (Must have Hospice experience) Alpha Comfort Care are seeking a compassionate and dedicated Lead Hospice Nurse, to join our team. In this role, you will provide high-quality nursing care to patients with terminal illnesses in their homes or hospice facilities. Your expertise will be essential in ensuring comfort and dignity for patients and support for their families during this challenging time. The ideal candidate will possess strong clinical skills, a deep understanding of end-of-life care, and the ability to work collaboratively within a multidisciplinary team. Duties Administer medications and manage pain relief protocols for patients. Provide comprehensive nursing assessments and develop individualized care plans. Deliver compassionate care to patients with dementia and other cognitive impairments. Assist patients with disabilities in daily living activities, promoting independence and dignity. Coordinate with physicians, social workers, and other healthcare professionals to ensure holistic patient care. Death pronouncement Patient visits Educate patients and families about disease processes, treatment options, and end-of-life decisions. Maintain accurate patient records using electronic health record systems such as Epic. Participate in quality improvement initiatives to enhance patient care outcomes (IDT). Offer emotional support to patients and families during the grieving process. Requirements Valid nursing license (RN) in the state of practice. Experience in home care, geriatric-hospice, or palliative care settings is required. Strong skills in medication administration and pain management techniques. Familiarity with caring for patients with dementia and working with individuals with disabilities. Knowledge of neonatal ventilator management is a plus. Experience in a Level I Trauma Center or sterile processing is advantageous. Background in senior care or ophthalmology can enhance patient interactions.
