Mercy Care Atlanta is seeking a compassionate anddetail-oriented Psychiatric RN Care Coordinator to enhance patientaccess to resources within our psychiatry practice. This role isessential in improving health outcomes for individuals receivingpsychiatric services by providing comprehensive care, medicationmanagement, and care coordination.

What You Will Do Conduct nursing assessments for patients entering the SubstanceUse Disorder (SUD) Program and coordinate with the supervisingphysician. Assist the psychiatry team by initiating medication therapy,including Long-Acting Injectables (LAIs), and facilitating thePatient Assistance Program (PAP) enrollment. Complete prior authorizations by liaising with insuranceproviders, pharmacies, and clients while maintaining meticulousfollow-up. Support providers by rooming patients, responding topsychiatric emergencies, and managing patient registries. Oversee medication reception and distribution in coordinationwith facilities management staff. Handle in-basket messages from providers, clients, and theaccess center, ensuring timely responses. Providing direct client care, performing mandatory clientscreenings, medication administration, monitoring responses to themedications and client education. Assist clients with linkage of care to other resources allowingthe clients to complete the healthcare continuum. Participate in community outreach, including health fairs andevents three times per year. Adhere to departmental policies and Mercy Care’s mission ofserving underserved populations.

Minimum Qualifications Associate’s or Bachelor’s Degree in Nursing. Active RN license and CPR/BLS certification. At least two years of related experience, preferably in anoutpatient care setting. Psychiatric experience and a passion for serving disadvantagedpopulations are highly desirable.
