Job Summary :

Uses the Nursing Process to plan, implement, direct, and evaluate patient care to achieve optimal patient outcomes. Works collaboratively with all other clinic personnel.

Education and Technical Skills:

Wound Care / Staff Nurse must have and maintain a valid CPR certification from the American Heart Association. Nurse must also have and maintain a valid Nurse Practitioner (NP), Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Georgia. Must have prior experience working with wound care patients.

Detailed Responsibilities:

Patient Case managing Coordinating Home Healthcare and DME suppliers Telephone support and troubleshooting patient issues between visits Evaluate effectiveness of plan/of care and modifies, as appropriate Involve patient/family/significant other in developing plan of care Recognize change in the patient’s condition and intervene appropriately. Conduct initial and ongoing wound measurements and assessments. Preparing wounds for procedures including cleansing and topical anesthetic application Assisting in surgical debridements and other procedures Applying wound dressings Applying compression wraps Assisting in cast applications Accounting for office and room supplies and equipment Contribute to Interdisciplinary plan of care using assessment data. Collaborate with other clinical disciplines in developing plan of care. Provide care based on scientific principles/nursing theory and evidence based practice. Prioritize nursing actions, reflecting patient needs, acuity, and desired outcomes. Implement physician orders accurately within appropriate time frames. Identify continuing care needs of the patient for the discharge plan. In the absence of a front desk coordinator, scheduling patients for wound care visit, utilizing the practice software systems as required.

Provide patient/family education to include:

Assessment of learning needs, abilities and readiness to learn, Documentation of teaching activities, Provision of instruction as appropriate, e.g. pain management, medication, diet, equipment, activities after discharge, Provision of appropriate educational materials, as available prior to discharge Document all care provided accurately & completely according to policy. HIPPA Compliance – complies with all of the Privacy and Confidentiality Policy and Procedures outlined in the “Privacy, Security and Confidentiality Agreement. Adheres to all Patient Safety Initiatives and Infection Control Procedures as outlined in the policy manual. Appropriate use of all resources supplies and equipment.
