Responsibilities The RN Case Manager provides intermittent skilled nursing services;communicates the patient’s progress with other disciplines anddirects, supervises and instructs hospice aide staff in theprovision of personal care to the patient. As a RN Case Manager youwill: Under the physician’s order, admit patients eligible forhospice services Assess and evaluates patient needs/problems, identifiesmutually agreed upon goals with patients Report patient status and need for other disciplines toclinical leadership, attending physician and hospice physician Update care plans on an ongoing basis; revise and resolvepatient problems and goals as changes occur and/orrecertification Complete informational visit and obtain patient consents forhospice admission per office procedure Be responsible to ensure the use of the 4Ms (What Matters tothe patient, Medications, Mentation, and Mobility) and provideAge-Friendly Care Qualifications Current unencumbered registered nurse in the state of practiceor in accordance with the Board of Nurse Examiners rules for NurseLicensure Compact (NLC) Must maintain a valid driver’s license and good drivingrecord Ability to work in a field setting and exhibited ability tomake sound nursing judgments Ability to assess patient needs and formulate individualizedpatient care plans to meet those needs #ghospice Pay Transparency Individual compensation packages are based onvarious factors unique to each candidate, including skill set,experience, qualifications, and other job-related considerations.
