GWINNETT COUNTY WE CURRENTLY HAVE A MINIMUM OF 20 CASES THAT NEED IN HOME VISIT EVERY OTHER WEEK.
Each visit takes about 15mins.
PAY RATE

Job Duties:
Evaluate, assess, and monitor the needs of members via f2f every 2 weeks Evaluate and assess CNA, PCA, and HHA as needed Inform members about any changes made at GHI Homecare regarding operations Report complaints or other issues Supervise management of records Directly reports to the Director. Ability to communicate effectively Establish a compassionate environment by providing support to member’s family Be able to provide assessments via EVV app Supervisory visits when necessary The above duties are not limited to and may include additional responsibilities as needed Located in DEKLAB COUNTY and surrounding areas

Conduct the initial and on-going in-home supervisory visits, evaluating the member needs, monitoring and supervising care, completing supervisory and members progress notes, making appropriate recommendations, reviewing and signing personal care support progress notes, educating members caregivers, providing staff training, supervising personal support aides GHI Lifesharing also known as GHI Homecare is a private-in home care company providing Medicaid approved unskilled nursing care through the Georgia Pediatric Program (GAPP). Personal Care Assistants provide quality nursing services by working with the patient care team to assess, plan, implement, and evaluate patients care and providing a positive, supportive environment. Our pediatric client has physical limitations but is highly intelligent. Clients may be diagnosed with conditions ie: TBI, muscular dystrophy, cerebral palsy, spina bifida etc.
Additional Requirements:
The ability to communicate effectively The working knowledge of the Rules and Regulations for Private Home Care Providers The working knowledge of the policies and procedures of the GAPP Medicaid Waiver Program CPR/First Aid certified Certificate showing Negative for TB Valid driver’s license Photo copy of Social Security Card Current proof of auto insurance Must have reliable transportation Certification/license in the practice of Nursing (RN ) Pass background check
