Full Job Description Overview:

We are seeking a dedicated and compassionate LPN/RN Assessment Case Coordinator to join our healthcare team. This role is responsible for providing comprehensive care to members, focusing on their physical, emotional, and social needs. The ideal candidate will have experience in home health care coordination, geriatrics and/or public health. Atlanta Pro Health is a Social Service Case Management agency focused on providing Service Coordination services to Medicaid recipients who are eligible for the Elderly and Disabled Waiver Program (EDWP). Our Care Coordinators works with adults to help them to navigate, understand, and secure the services needed in order to live independently in their home and community. This position is responsible for the initial assessments and annual reassessments determining the Level of Care according to the EDWP guidelines, developing and implementing of individualized care plans, reassessing members needs due to condition changes; monitoring and evaluating of all clinical outcomes to ensure that services are provided in a professional and comprehensive manner.

Responsibilities:
  • Conduct thorough in home client assessments to identify potential clients needs and develop personalized care plans.
  • Collaborate with healthcare providers to coordinate care and services.
  • Review medications and treatments as prescribed by physicians.
  • Assist with coordinating care, including developing care plan and discharge planning and facilitating their services in home care.
  • Develop and implement discharge plans, ensuring that clients receive the necessary support and resources to allow them to remain in their home/community.
  • Provide education and support to clients and their families encouraging healthy habits and disease prevention.
  • Document client information and care plans in a timely and accurate manner.
  • Utilize electronic medical records to track client progress and update care plans accordingly.
  • Responsible for knowing and understanding the EDWP policies and procedures.
Duties:

Schedule appointment for face-to-face interview with the client at his or her residence or other appropriate site. Collect medical documentation which supports a nursing home level of care need. Analyze and interpret all medical, social information as compiled, and obtain additional information as needed. Conduct in home assessments face-to-face with the client and/or authorized representative using the intermediate level of care criteria and the MDS-HC. Obtain client signatures for required forms. Develop the Comprehensive Care Plan to reduce the risks identified in the MDS-HC assessment. Complete assessments on clients following EDWP guidelines and submit all assessments to a supervisory RN for review and approval. LPN makes corrections and additions as recommended by the supervising

will serve geographic areas which may include parts of one large county and/or many small counties which involve extensive travel.

Qualifications:
  • Current valid LPN/RN license in the state of Georgia
  • Minimum Data Set (MDS) Assessment Training
  • Experience in home health care, hospital and inpatient settings.
  • Familiarity with geriatric care and ability to work effectively.
  • Strong communication and interpersonal skills, with the ability to work collaboratively with clients, families, and healthcare providers.
  • Ability to work independently and make sound decisions.
  • Experience with discharge planning and client education.
  • Knowledge of electronic medical records and ability to navigate them efficiently.
