Job Description

PRN Nurse Practitioner Hospice (Columbus, GA.) – ProHealth Home Health & Hospice 4.1 4.1 out of 5 stars Columbus, GA 31904 Duties and Responsibilities 1. Assist with oversight of the implementation of the entire physician, nursing, social work, therapy, and counseling areas within Hospice to ensure that these areas consistently meet patient and family needs. 2. Assist with responsibility for the medical component of the Hospice’s patient care program. 3. Conduct Face-to-face encounters as part of the certification process, but cannot certify the terminal illness for a hospice patient. This includes but is not limited to: Visit hospice patients in the private home, assisted living facility, nursing facility, etc to perform an actual physical encounter with the hospice patient to perform a complete head to toe physical assessment, document the findings on agency required forms and them communicate those finding to the attending physician and Hospice Medical Director as part of the certification/ recertification process. This face-to-face visit will be performed on all hospice patients entering their 3rd or greater election period. Participates in and acts as a medical resource to the IDG and Hospice leadership. 5. Participates in the establishment and implementation of the plan of care, which is coordinated with the attending physician, Hospice Medical Director, and IDG to review and update the place of care when changes are made and at least every fifteen days. 6. Consults with attending physicians, if requested. 7. Is available to patients on a 24-hour basis to manage their terminal illness and medical needs to the extent that the attending physician or Hospice Medical Director is absent or not able to meet these needs. 8. Acts as a liaison with other physicians in the community services by Hospice and facilities communication. 9. Participates in educational programs for staff, when requested. 10. Is a member of, communicates, and participates in designated interdisciplinary group activities. 11. Provides advice, guidance, and assistance to Hospice staff until a satisfactory resolution is reached when a medical order: Is of a questionable nature. Contains a discrepancy. Lacks clarity. Continues to be a concern for the staff after consultation with the primary physician and/ or Hospice Medical Director. 12. Provides medical consultation and direction when the attending physician or their design cannot be reached and there is a change in the patient’s condition requiring medical attention. 13. Respect the patient’s confidentiality and choices. 14. Provision of consultation on specialty populations.
