Skilled Nursing Intake/Admission Review Nurse Paulding Care Center for Nursing and Rehabilitation 600 West Memorial Drive, Dallas, GA 30132 The review nurse plays a vital role in determining if a patient meets the criteria for skilled nursing services. Responsible for reviewing patient information to facilitate appropriate placement and ensuring a smooth admission process. Main responsibility includes reviewing clinical documentation. Assess medical records, physician orders, and other relevant information to determine the level of care required and if the patient is suitable candidate for skilled nursing based on admission criteria. This includes evaluating the medical necessity of care. The role of a skilled nursing admission review nurse includes bridging the gap between the initial referral and the patient’s admission to our skilled nursing facility. To ensure that patients who require skilled nursing services are appropriately placed and that the transition process is as seamless as possible for all parties involved. Must have strong clinical background and computer skills. Able to collaborate with hospitals, physicians, and other clinical professionals. Must be able to communicate effectively within the facility to ensure all necessary medical equipment and supplies are in place for all expected admissions.
