Responsibilities:

• Provide direct patient care, including assessing and monitoring patients’ conditions, administering medications, and performing treatments
• Collaborate with healthcare team members to develop and implement patient care plans
• Document patient information, including medical history, symptoms, and treatment plans
• Educate patients and their families on healthcare procedures, medications, and self-care techniques
• Ensure compliance with infection control standards and maintain a safe and clean working environment
• Assist with the coordination of patient referrals and follow-up care
• Respond to emergencies and provide appropriate interventions

Experience:

• Minimum of [1] years of experience as a Nurse in a clinical setting
• Experience in health coaching and providing education to patients and their families
• Familiarity with medical records documentation and electronic health record systems
• Knowledge of dementia care techniques and protocols for working with elderly patients
• Experience in geriatric nursing is a plus
