Hospice RN Case Manager Hillcrest Hospice & Palliative Care Conyers, GA 30012 Job Summary The Registered Nurse Case Manager plays a crucial role in coordinating patient care and ensuring the delivery of high-quality healthcare services. This position involves working closely with patients, families, and healthcare teams to develop and implement individualized care plans. The ideal candidate will possess a strong background in managed care, geriatrics, and medical-surgical nursing, with a focus on providing exceptional patient service in various settings, including clinics and Level I Trauma Centers. Duties Assess patients’ needs and develop comprehensive care plans tailored to individual requirements. Collaborate with interdisciplinary teams to ensure effective communication and coordination of care. Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes. Provide education and support to patients and their families regarding treatment options, health management, and resources available. Conduct regular follow-ups to evaluate the effectiveness of interventions and make necessary adjustments. Maintain accurate documentation of patient interactions, care plans, and progress notes in compliance with regulatory standards. Advocate for patients’ needs within the healthcare system to ensure they receive appropriate services. Stay informed about advancements in nursing practices, particularly in geriatrics and physiology knowledge. Qualifications Current Registered Nurse (RN) license in the state of practice. Experience in managed care settings preferred; familiarity with Level I Trauma Centers is a plus. Strong knowledge of geriatrics, medical-surgical nursing, and operating room (OR) procedures. Excellent patient service skills with a compassionate approach to care. Ability to work collaboratively within a multidisciplinary team environment. Strong organizational skills with attention to detail for documentation and reporting purposes. Effective communication skills, both verbal and written. Basic computer proficiency for electronic health record management.
