Schedule:
M-F 8a-4:30p. PRN minimum requirement is 10hours/month.

JobSummary:
Coordinates, facilitates and performs interdisciplinaryplan of care through assessment and evaluation of patient conditionand effectiveness of care under the direction of the attendingphysician by traveling to patient homes, Community-BasedResidential Facilities (CBRF), assisted living locations, hospitalsor other to meet patient and family/caregivers, assess patient careneeds, acuity in relation to policies, procedures and regulationsof home care and/or hospice services.
Job Responsibilities andRequirements:
PRIMARY RESPONSIBILITIES
Assesses and coordinates patient’s discharge planning needswith members of the healthcare team. May perform point of care testing according to policies andprocedures. Collaborates, consults and communicates with physicianregarding the plan of care and changes to patient status. Providesphysician with regular updates on progress and recommends revisionof plan care, documents status and communication in adherence withregulations. Coordinates and communicates with multi-disciplinary teammembers and outside agencies as appropriate to achieve expectedoutcomes for patient care and education through end of life ordischarge, ensuring accurate and complete documentation andcoordination of unique care plan. Completes admission paperwork and verifies insuranceinformation. Completes a comprehensive medical assessment byentering data into the clinical computer program. Ensures thatpatient assessment is completed in a timely manner. Provides skilled nursing care within the established plan ofcare, including performance of tasks such as taking vitals,performing wound care, IV care, ostomy care, blood draws,injections, and the physical assessment of the overall patient’smedical condition. Documents visit notes, treatments, and care. Follows visitpattern according to physicians orders and ensures required visitsare completed in a timely manner. Ensures all documentation istimely, accurate and complete in adherence to regulations. Collaborates with the patient, patient’s family and healthcareteam to develop a comprehensive plan of care based on the uniqueneeds of the patient. Responds to calls from patients, familymembers and others, completes follow-up to address needs, issues,or concerns. Works in a constant state of alertness and safe manner. Performs other duties as assigned. #

Graduate of accredited school of nursing or educationequivalency for licensing

PHYSICAL REQUIREMENTS
Constant use of speech to share information through oralcommunication. Constant standing and walking. Frequent lifting/carrying and pushing/pulling objects weighing0-25 lbs. Frequent use of vision and depth perception for distances near(20 inches or less) and far (20 feet or more) and to identify anddistinguish colors. Frequent use of smell to detect/recognize odors. Occasional lifting/carrying and pushing/pulling objectsweighing 25-50 lbs. Occasional lifting/moving of patients.
