Job Overview:

Park Place Nursing and Rehab Facility is seeking a compassionate and experienced Wound Care Nurse to join our dedicated clinical team. This role is vital in supporting our residents’ healing and quality of life through expert wound management in a long-term care setting. The ideal candidate will bring clinical skill, attention to detail, and a strong commitment to patient-centered care in a nursing home environment.

Key Responsibilities:

Conduct comprehensive skin and wound assessments for residents, with routine monitoring and follow-up. Develop, implement, and revise individualized wound care plans in collaboration with the interdisciplinary care team. Administer wound care treatments such as dressing changes, pressure injury prevention, debridement, and infection control measures. Participate in weekly wound rounds and maintain detailed, accurate documentation of wound progression. Educate residents, families, and staff on proper wound care techniques and prevention strategies. Collaborate closely with nursing staff, attending physicians, dietitians, and therapy teams to ensure optimal healing outcomes. Monitor resident condition changes and promptly report concerns to medical staff. Support the clinical team with emergency response and basic life support as needed.

Qualifications & Skills:

Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Georgia Wound Care Certification (WCC, WOCN, or equivalent) preferred Minimum 1-2 years of wound care experience, preferably in a skilled nursing or long-term care setting Strong understanding of pressure injury staging, wound vacs, and infection prevention Excellent communication and interpersonal skills for working with residents, families, and staff Commitment to delivering high-quality, resident-centered care in accordance with facility protocols and regulatory standards Why Join Us?
