Key Responsibilities:

Conduct in-home visits, health assessments, and develop care plans for patients with acute and chronic conditions Collaborate with interdisciplinary care teams to deliver high-quality, patient-centered care Participate in a shared rotation for after-hours telephonic on-call coverage

Requirements:

Bachelor’s Degree in Nursing with an active Georgia license (Master’s preferred) Board Certification for Nurse Practitioners and DEA Licensure Valid Driver’s License and Proof of Auto Insurance Apply Today!
