Assumes fullnursing responsibility for the delivery of the Plan of Care foreach hospice patient. Continuously evaluates personal andprofessional performance and makes necessary changes to increaseproductivity and quality of care delivered. Makes the initial nursing evaluation in determining eligibilityfor hospice services during visit within forty-eight (48) hours ofreferral. Identifies the patient’s/family’s physical, psychosocial, andenvironmental needs and re-assesses as needed, no less than everyfifteen (15) days. Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, andpatient/family response to hospice care. Collaborates with the patient/family, attending physician andother members of the IDG in providing patient and family caredaily. Instructs and supervises the patient/family in self-caretechniques when appropriate. Maintains accurate and relevant clinical notes regarding thepatient’s condition. Experience Requirements One year of clinical experience. License Requirements Current RN licensure in the state of practice and one year ofclinical experience. Current CPR Certification. Current driver’s license, valid vehicle insurance, and accessto a dependable vehicle, or public transportation.
Additional State Specific Requirements LA:
a minimum two years full time experience working as an RN.However, two years of full time clinical experience in hospice careas a licensed practical nurse may be substituted for the requiredtwo years of experience as a registered nurse.
