Job Summary The Registered Nurse in Home andCommunity Based Services provides and directs provisions of nursingand personal care to patients in their homes as prescribed by thephysician or as authorized by the state/program and in compliancewith applicable laws, regulations, and agency policies. TheRegistered Nurse coordinates the plan of care with other healthcare professionals involved in care and helps to achieve andmaintain continuity of patient care by planning and exchanginginformation with physician, agency personnel, patient, family, casemanager and other community resources. Specific JobDuties/Responsibilities

• Provides high quality clinical serviceswithin the scope of practice and within infection controlstandards, in accordance with the plan of care/service plan, and incoordination with other members of the patient/client’s care teamfrom admit through discharge.
• Completes clinical nursingassessments in accordance with federal and/or state programrequirements and as required by payer.
• Ensures thepatient/client’s eligibility and medical necessity for services asdefined by payer source and agency policy.
• Develops and revisesindividualized plans of care and/or service plans according tofederal and/or state program requirements with other communityproviders.
• Ensures plan of care frequency and duration meets theneeds of the patient and initiates plan of care revisions as neededwith physician approval. Experience Desired
• A minimum of one yearexperience as an RN preferred License Requirements
• Must havecurrent RN licensure in state of practice.
• Current driverslicense, vehicle insurance and access to a dependable vehicle orpublic transportation.
• Current CPR certification required. StateSpecific Requirements

At a minimum, one year of clinicalexperience as a Registered Nurse. One year of clinical experienceas a Registered Nurse may be waived for a Registered Nurse withrecent clinical experience as an LPN. RN licensure must have norestrictions
