Home Health Registered Nurse Clarity Home Health Baton Rouge, LA Clarity Home Health is a locally owned and operated full-service home health agency that operates in Baton Rouge and the surrounding areas. Clarity Home health is growing and we are currently hiring Field RNs effective immediately. We aim to provide outstanding, high quality healthcare to each of our patients with the time and attention necessary to meet his or her needs in their home. Qualifications

• Active Louisiana RN License
• Home Health experience preferred but not necessary, minimum one year hospital experience required per State law
• Have a reliable vehicle with active driver’s license and insurance
• Current CPR certification If No Home Health Experience A minimum of 1-year nursing experience and strong nursing skills.

You must be able to multi-task and prioritize your patients needs independently and instruct patients and/or caregivers concerning disease processes and medications. Proficient with nursing skills, such as venipuncture, wound care, urological procedures, infusion and other skills to meet the patients needs following medical orders. Requirements and Responsibilities

• RNs provide an initial, resumption, and re-certification evaluation of the Home Health patient in their home setting utilizing EMR (Wellsky).
• Coordinates the initial disciplines on the plan of care and throughout the patient episode, collaborating with the physician, patient, family member and team manager to develop the expected patient visits.
• Documents all care for home health patients. Performs skills as ordered on the plan of care. Maintaining continuity of care from admission to discharge
• Communicates changes in the condition and needs of the patient with all team members including RNs, LPNs, Therapists, Social Workers, Home Health Aides, Physician, patient, family, and other individuals to ensure that the patient’s plan of care is appropriately developed.
• Understands and adheres to established Agency policies and procedures
