Pinnacle Hospice is currently recruiting for a full-time General Inpatient Care RN to oversee the patient care and service coordination of our patients requiring general inpatient care. The position duties include the following: Collaborating with doctors and hospice team members to develop the plan of care. Making routine visits and completing physical assessments. Working with caregivers on discharge coordination, if applicable. Providing emotional and psychosocial support to patients and their loved ones. This position will work daily with the hospice medical director team and interdisciplinary team members to provide high acuity end of life care. Hospice experience is required and acute facility experience is strongly recommended.
