The ideal candidate will have experience in home health. As a Case Manager, you will play a vital role in coordinating patient care and communicating with patients, staff and referral sources while ensuring that patients receive the necessary support throughout their treatment journey. Responsibilities Consult with Director and RN regarding admissions and schedules. Collaborate with healthcare providers, including nurses and specialists, to coordinate acute care services. Maintain hospital and lab board to ensure that all ordered labs are obtained and faxed to MD. Facilitate communication between patients, families, and healthcare teams to ensure continuity of care. Monitor charting and perform QA. Manage fax for referrals, orders, labs and other communications. Order and maintain wound care supplies. Advocate for patients’ needs within the healthcare system to ensure they receive appropriate services. Observe confidentiality and safeguard all patient information. Qualifications Previous experience in home health. Excellent communication skills for interacting with patients, families, and multidisciplinary teams.
