Discusses alternative care options with patient / caregivers aswell as the multi-disciplinary team and assists with dischargeplanning needs. Facilitates movement along the health carecontinuum to ensure quality, cost-effective outcomes are achievedin collaboration with the multi-disciplinary team.

  • Graduate of an accredited schoolof nursing.

Preferred

  • Bachelor’s degree in nursing.

CertificationsRequired

  • Current registered nurse (RN)license in state of practice.

Basic Life Support (BLS.) through theAmerican Heart Association.

Preferred

  • -Certification in CaseManagement (CCM).

Health Care Management (CPHM).Knowledge Skills andAbilities (KSAs) Proficiency in using computers, software, and web-basedapplications. Effective verbal and written communication skills and abilityto present information clearly and professionally to varying levelsof individuals throughout the patient care process. Good organizational and time management skills. Conflict resolution skills and ability to demonstrate goodjudgement and decision making in performing tasks.

Job Duties Manages various aspects of the planning process in anticipationof the patient’s movement through the healthcare system. Uses care managements policies and procedures includingapplication of evidenced based criteria and level of care andcontinued appropriateness. Manages the care of assigned patients through the healthcaresystem based on the patient’s individual needs. Prepares, executes, and reinforces post-discharge careplan. Adapts behavior to the specific patient population, includingbut not limited to respect for privacy, method of introduction tothe patient, adapting explanation of services or procedures to beperformed, requesting permissions and communication style. Performs other related duties as required.
