This job functionsas the liaison and communicator with the patient, caregivers,healthcare providers, and multi-disciplinary team members as wellas post-acute care and third party payers. Discusses alternativecare options with patient / caregivers as well as themulti-disciplinary team and assists with discharge planning needs.

  • Graduate of an accredited school of nursing. Preferred
  • Bachelor’sdegree in nursing. Work Experience Required
  • 2 years of relatedexperience. Certifications Required
  • Current registered nurse (RN)license in state of practice. Basic Life Support (BLS.) through theAmerican Heart Association. Preferred
  • Certification in CaseManagement (CCM). Health Care Management (CPHM). Knowledge Skillsand Abilities (KSAs)
  • Proficiency in using computers, software,and web-based applications.
  • Effective verbal and writtencommunication skills and ability to present information clearly andprofessionally to varying levels of individuals throughout thepatient care process.
  • Good organizational and time managementskills.
  • Conflict resolution skills and ability to demonstrategood judgement and decision making in performing tasks. Job Duties
  • Manages various aspects of the planning process in anticipationof the patient’s movement through the healthcare system.
  • Usescare managements policies and procedures including application ofevidenced based criteria and level of care and continuedappropriateness.
  • Manages the care of assigned patients throughthe healthcare system based on the patient’s individual needs.
  • Prepares, executes, and reinforces post-discharge care plan.
  • Adapts behavior to the specific patient population, including butnot limited to respect for privacy, method of introduction to thepatient, adapting explanation of services or procedures to beperformed, requesting permissions and communication style.
  • Performs other related duties as required.
