Full Job Description Duties:

• Assess and evaluate patient needs and develop individualized care plans
• Coordinate and facilitate communication between patients, families, and healthcare providers
• Monitor patient progress and make necessary adjustments to care plans
• Provide education and support to patients and their families regarding their healthcare needs
• Collaborate with interdisciplinary healthcare team members to ensure comprehensive and coordinated care
• Document patient assessments, interventions, and outcomes accurately and timely
• Adhere to all applicable laws, regulations, and standards of practice, including

HIPAA Requirements:

• Valid Registered Nurse (RN) license
• Minimum of 2 years of nursing experience in hospice required
• Knowledge of physiology and ability to interpret vital signs
• Strong communication and interpersonal skills
• Ability to work independently as well as part of a team
• Excellent organizational and time management skills
• Knowledge of HIPAA regulations and ability to maintain patient confidentiality
• Commitment to providing exceptional patient service
