This job functionsas the liaison and communicator with the patient, caregivers,healthcare providers, and multi-disciplinary team members, as wellas post-acute care and third party payers. Discusses alternativecare options with patient/caregivers as well as themulti-disciplinary team and assists with discharge planning needs.

- Graduate of an accredited school of nursing. Preferred
- Associate’s or Bachelor’s degree in nursing. Work ExperienceRequired
- 2 years of nursing experience. Certifications Required
- Current registered nurse (RN) license in state of practice. BasicLife Support (BLS) from the American Heart Association. Preferred
- Certification in clinical specialty area. Knowledge Skills andAbilities (KSAs)
- Proficiency in using computers, software, andweb-based applications.
- Effective verbal and writtencommunication skills and ability to present information clearly andprofessionally to varying levels of individuals throughout thepatient care process.
- Excellent conflict resolution skills.
- Ability to work a flexible work schedule (e.g. 24/7, weekend,holiday, on call availability) and travel throughout and betweenfacilities. Job Duties
- Provides continuity of care by ensuringsmooth transitions between care settings.
- Develops relationshipswith patients and their multidisciplinary team to facilitate and/ornavigate through subsequent treatment and follow-up to reflectcontinuity of care.
- Manages high risk, complex patient care withthe goal of minimizing readmission.
- Works closely with physiciansto coordinate patient’s care plan communication; works withmultidisciplinary team to maintain and implement up-to-datecoordinated patient centered care plan; communicates with allmembers of the healthcare team as patient advocate.
- Prepares,executes, and reinforces post-discharge care plan.
- Identifiesbarriers to care in an effort to elicit changes in processes forpatients navigating the continuum of care.
- Collaborates withleadership to review processes with the goal of improving theclinical experience for referred patients and the referringphysician.
- Adapts behavior to the specific patient population,including but not limited to respect for privacy, method ofintroduction to the patient, adapting explanation of services orprocedures to be performed, requesting permissions andcommunication style.
- Performs other related duties as required.
