Job Description

Registered Nurse – State Home Care Program .Part-time and Full-timepositions available , FT = 35 hours/week NOT remote Responsible forassigned nursing duties for Clinical Assessment and Eligibility(CAE)/State Home Care Program. Will Perform Coordination of Carescreenings to determine medical eligibility and all duties requiredunder the Coordination of Care Program and Waiver Program. Completeinitial assessments and annual reassessment visits on all personal careand home health aide clients, interim visits as needed. Developappropriate care plans for personal care clients. Prepare and maintainnursing paper and electronic files. Provide consultation on ProtectiveServices, Elder at Risk and Respite Care cases. Act as a liaison withagency providers. Act as resource person for medical matters to allGreater Springfield Senior Services staff. Act as a backup to the nurseof any other medical programs. Active member of the interdisciplinaryteam providing nursing consultation to case management staff. Advocateand act as a resource in areas of interest and/or concern for elders.

Assist with information and referrals. Requires valid driver’s licenseand reliable transportation. Must be able to climb multiple flights ofstairs Education and Experience Graduate from a school of nursing schoolaccredited by the National League for Nursing A Bachelor of Sciencedegree in Nursing or an Associates of Science Nursing degree in Nursingand at least one (1) year clinical experience and/or nursing in thecommunity or long-term care setting. Or A diploma RN and two (2)additional years of community-based nursing experience or long-term caredirect service experience. Core Competencies Ability to utilizetechnology in the field to complete On Site Assessments includingLaptops, Tablets, Personal Hot Spots Devices, WIFI, etc. Workingknowledge of spreadsheets, word processing and data entry, includingWindows, Word, and Excel helpful Ability to conduct professionalassessments in a variety of home settings in a manner that is respectfuland supportive to the occupants of the home Ability to work with personswith diverse backgrounds, skills and abilities Experience working withdisabled persons preferred All staff are expected to pursue and/orenhance personal cultural competence through education, training, and anopenness to understanding all cultures. Must be able to climb multipleflights of stairs Travel reimbursement is $0.62/mile
